Death Certificate
Overview
The Death Certificate screen is used to collect data needed for a customer’s initial arrangement appointment and the vital statistics required for the state's death certificate.
The amount of information entered on this screen varies, depending on your preferences and requirements. Some fields are automatically populated by information already entered elsewhere in the application, such as the First Call screen. It is important to select the correct Death Certificate STATE, as all areas and associated fields of this screen are state specific. Therefore, only fields that are valid for the Death Certificate State selected are displayed.
Some fields on the Death Certificate screen have logic defined for them. For example, if you choose "Residence" in the Place of Death Type field, the system will copy the decedent's address information to the place of death fields. For example, Address, City, State, etc..
The Death Certificate screen can be accessed from the menu bar by navigating to Cases > Vital Statistics > Death Certificate .
Workflow
To add a new Death Certificate:
- Select the case you are working with from the dashboard
- Navigate to the Cases – Vital Statistics - Death Certificate screen
- If this is the first time to the screen, the Death Certificate state will automatically be set to the state that is on the Location for the case. You can change this if needed by selecting a state from the drop down list.
- The Death Certificate screen is one of the largest screens in the application; a lot of data is collected here. Go through the screen and fill in all fields that are required.
- Review any fields that have been automatically populated from other screens.
- Click Save when data entry is complete (you can always change the data later if desired) .
- To Print the death certificate, navigate to the Custom Documents & Forms widget on the Case Summary screen
- You can also go to the Custom Documents & Forms screen and choose the State DC form you wish to print.
Important Notes:
- The Death Certificate screen is available for both At Need and Pre Need cases.
- If you clear out the Death Certificate State field, ALL possible DC fields will display on screen. You can only view these, you cannot save a Death Certificate screen with a blank state; it is a required field.
